Basic Life and Accidental Death and Dismemberment (AD&D) Insurance

Underwritten and administered by New York Life, Active Day provides all active benefits-eligible employees with a basic life and AD&D benefit of $15,000.  This coverage is provided to you at no cost.  Please note we required all employees to complete the beneficiary designation form.

Supplemental Life Insurance

Underwritten and administered by New York Life, all benefits-eligible employees are eligible to participate in the Supplemental Employee, Spouse & Child Life Insurance plans.  Since this plan is voluntary the employee is responsible for 100% of the premium.

Short-Term Disability Insurance

Underwritten and administered by New York Life, Active Day offers full-time employees the opportunity to elect Voluntary Short-Term Disability (STD) income benefits.  Because this benefit is optional, the employee is responsible for the cost.
